Evaluation of Knowledge, Attitude and Practice of Healthcare Workers Regarding Human Immunodeficiency Virus

Human immunodeficiency virus (HIV) infection has been linked to more than 35 million deaths globally, and the levels of infection are increasing; thus understanding people's knowledge, attitudes, and practice (KAP) concerning HIV/AIDS is a critical component of the battle against the disease. Objectives: measuring healthcare workers’ knowledge, attitude, and practice regarding HIV/AIDS, and people living with HIV (PLHIV). Methods: A quantitative cross-sectional study was conducted in the western area of the Kingdom of Saudi Arabia (KSA) from September 2022 to October 2022. Results: The study included 263 healthcare workers from eight hospitals. The level of knowledge was excellent among 46.4%, good among 17.1%, fair among 12.9%, and poor among 23.6%. Most respondents' overall attitude was positive, while the practice pattern was poor. Good KAP was significantly associated with physicians, nurses, and previous experience with HIV patients. Conclusion: The level of knowledge was high, the attitude was positive, and the practice was poor among most respondents regarding HIV/AIDS with a significant positive correlation with being a health care physician or nurse, and having previous experience with AIDS cases.

Melanoma Associated Leukoderma: Case Series and Literature Review

Vitiligo is an acquired achromia linked to an autoimmune destruction of melanocytes. One of its mysterious aspects is its occurrence with melanoma known as melanoma-associated leukoderma (MAL). The Objective of the study is to shed the light on the clinical aspects of MAL for a better understanding while providing a comprehensive review of the literature. We retrospectively analysed the clinical characteristics of 12 patients having MAL, from 2016-2021. We compared our findings to those reported in the literature. Our series illustrates different situations where vitiligo is linked to melanoma. None of our patients had a positive family history of vitiligo. The median age was 68 years with extremes of 90 and 36 years, 10 patients had their MAL located on photo-exposed areas. Clinically MAL presented as diffuse, macular achromic patches located primarly at sites distant from the primary melanoma and notably on the trunk, legs and face with a late age of onset. No histological particularities as opposed to vitiligo were found. Given the clinical similarities of these achromias with conventional vitiligo, a more thorough clinical examination for melanoma in patients with vitiligo seems to be crucial. Special attention is needed for older patients presenting with late onset, very progressive vitiligo-like lesions refractory to standard treatment.

The Hidden Cardiac Amyloidosis and Red Flags: Case Report

It is imperative that internists and hospitalists who are the frontier of care seen by the patient are aware of the red flags associated with cardiac amyloidosis. This case report discusses the clinical evaluation of cardiac amyloidosis highlighting the utility of various laboratory tests and imaging modalities utilized to facilitate reaching a diagnosis. Herein, we discuss the clinical evaluation of a 69-year-old male with suspected cardiac amyloidosis highlighting the utility of various laboratory tests and imaging modalities to reach a diagnosis. Additionally, we discuss the pitfalls of delayed diagnosis.
